Study Termination

This Study Termination Request Form authorizes Medidata to terminate the single Study Sales Order, as may be amended by a Change Order (“Single study SO”) and the services for the corresponding study as indicated on this form. Expect as set forth herein, all terms and conditions of the Agreement between Medidata and Customer or Partner, as applicable (herein collectively referenced to as “Client”) shall apply to this request.

“Anniversary Day” means either: (i) the day following the day of the month of the Single Study SO end date, where the end date is other than 28th – 31st of the month; or (ii) the first day of month, where the Single Study SO end date is 28th-31st of the month.

Requests for study termination CANNOT be backdated

Requested Termination Date (must be on an Anniversary Day of the Single Study SO, at least three (3) months after the Date of Request; in cases where the date submitted does not reflect an Anniversary Day, the date of termination will be set at the next Anniversary Date) (MM/DD/YYYY).
